Issues like ping, lag, and weapon type are big factors in shooting to kill. However, when you shot first with a good weapon and low ping, yet your opponent still gets the drop on you then it likely boils down to aim.
Medal of Honor has a sophisticated algorithmatic system to calculate motion, range, and accuracy for each weapon. The system is vulnerable to flaw, nonetheless, it is a surprisingly effective system. Accuracy of aim does count, and it counts for a lot.
